Licensed Vocational Nurse

Monday - Friday 10am-6:30pm

OR

Tuesday - Saturday 8am-4:30pm

 

WHAT YOU WILL DO: This position provides support to address the medical needs of patients that have been discharged from Veterans Affairs (VA) Medical Center, Palomar Hospital and Tri-City Hospital, to achieve a more adaptive level of physical, psychological and social functioning while in the program. Working within a comprehensive team of skilled professionals the Licensed Vocational Nurse engages with the recuperative care team to get the patient back to self-sufficiency.  

 

• Conducts daily rounds, checks vital signs, and evaluates patient progress

• Conducts weekly medication management with each patient; this includes organizing medications in weekly pill organizer, ensuring patients are taking medications as prescribed, educating patients on purpose of medications and side effects, and assisting with prescription refills as necessary

• Conducts weekly compliance check with each patient by searching contents of the medication box, ensure a current medication list is present, and that the medication box is locked

• Assists patients in checking blood glucose levels and ensures proper methods of self-administering insulin injections

• Promotes patients' self-determination by establishing individual care goals and teaching patients to understand conditions, medications, and self-care skills

• Assists with wound care, dressing changes, and catheter care on an as needed basis

• Educates patients on medical conditions and treatment compliance, including diet and smoking cessation

• Assists with trainings and educates staff and patients on various medical-related topics

• Participates in regular administrative and patient case file review meetings

• Communicates medical concerns or special needs related patients' physical health to Recuperative Care staff

• Maintains an active file on all clients including the documentation of vital signs, medication box check logs, and progress notes

• Prepares rooms and ensures that stock of supplies is maintained

• Provides guidance to Certified Nursing Assistants, interns and volunteers

• Ensures the confidentiality of patient information and health records

• Provides medical oversight for individual patient cases through regular communication with medical providers and review of medical notes

• Assists patients with making medical appointments and any transportation necessary

• Participates in all staff meetings and trainings

• Performs additional duties as assigned

 

Requirements

 

EDUCATION / EDUCATION

• Must be currently licensed by the State of California as a Licensed Vocational Nurse

• 1 + years nursing experience required

• Knowledge of alcohol and other drug recovery field preferred

• Experience working with severely mentally ill, medically fragile, and homeless populations preferred

• Basic Life Support (BLS) for healthcare providers (American Heart Association preferred) to be completed within three (3) months of hire
